Prior laboratory research experience
Bachelor's degree from a regionally accredited US institution, or a comparable degree from an international institution
General Graduate Record Exam (GRE) Scores
- Official scores must be reported directly from the ETS testing center to UW-Madison (institution code 1846)
- GRE subject test is recommended but not required
Test of English as a Foreign Language (TOEFL) Scores
- Official scores must be reported directly from the ETS testing center to UW-Madison (institution code 1846)
- Required of all applicants whose native language is not English OR whose undergraduate instruction was not in English

Applicants are exempt from the TOEFL if:
- The official language of instruction at the undergraduate level is exclusively English
- They hold a degree from an accredited American college or university
- They have completed and received grades for 2 semesters of course work at an accredited American college or university or an international university where English is the exclusive language of instruction
